2018 MAZDA CX-9

Filed Jul 15, 2026

The cylinder head on this 2018 Mazda CX-9 (Skyactiv-G 2.5T engine) cracked, allowing engine coolant to leak and intermix with engine oil, causing loss of coolant and catastrophic engine failure. The vehicle became undriveable in the first week of November 2025 at approximately 125,019 miles and has remained inoperable since; it was not in a crash and had to be towed. This matches a known, documented defect on Mazda's Skyactiv-G 2.5T engine: coolant leakage at the cylinder head near the exhaust…

This complaint refers to Mazda and it's dealer refusal to properly execute the warranty work under the SSPD5 Valve Stem Seal Class Action Campaign and does not follow the Court's approval of the final Settlement Agreement. I assert that Mazda and it's dealer are not following the SSPD5 repair procedures. 1. I have documentation and complaints going back to Dec-2023 with this particular dealer that I have experienced low level oil consumption within the 7,500 mile oil change interval. The dealer…

2019 MAZDA CX-9

Filed Jul 10, 2026

The engine cylinder head developed an internal coolant leak, allowing coolant to enter the combustion chamber. This caused engine overheating and smoke coming from the tailpipe while driving. A dealer performed a coolant pressure test confirming coolant leaking into the combustion chamber, and wet/dry compression testing on all 4 cylinders showed low and inconsistent readings consistent with cylinder head coolant intrusion (Cylinder 1: 105/125 psi, Cylinder 2: 115/125 psi, Cylinder 3: 110/175…

2011 MAZDA CX-9

Filed Jun 30, 2026

The front driver-side lower ball joint suffered a catastrophic structural failure and sheared completely apart while moving. This vehicle was previously serviced under Safety Recall Campaign 15V451000 / Mazda Recall 8515G for this exact defect. The recall remedy part failed a second time, collapsing the front wheel assembly, ripping the CV axle completely out of the transmission casing, causing a hazardous transmission fluid spill, and leaving the vehicle stranded and immobilized in the middle…

Crash

The contact owned a 2020 Mazda CX-9. The contact stated that while driving approximately 60 MPH, a second vehicle crashed head-on into the front of the vehicle. During the head-on crash, no air bags inside the vehicle deployed. The contact sustained a concussion and right eye injury and later received medical attention. A Police report was filed at the scene. The vehicle was destroyed and towed to a local storage facility. No information was available concerning the second vehicle's occupants.…

The mazda Cx-9 is having the same fuel pump issues that was recalled up until 2018 models. The issue with the lean codes and faulty fuel pump continues

2019 MAZDA CX-9

Filed Jun 18, 2026

My 2019 Mazda CX-9 (VIN [XXX]) suffered catastrophic cylinder head failure on the SKYACTIV-G 2.5T engine at approximately 124,000 miles. The failure is consistent with Mazda TSB 01-013/21 (and successors 01-007/22, 01-010/24), in which Mazda acknowledged that cylinder heads on this engine can crack at the stud bolt hole and exhaust manifold flange due to manifold deformation and residual production stress. The first warning came in mid-May 2026, when the engine temperature warning illuminated…

2021 MAZDA CX-9

Filed Jun 18, 2026

Check engine light comes on and error P0303 comes on. Had an independent mechanic do the following and after each one, error P0303 still came on 1. Swap the ignition coils from 2 to 3 2. Swap spark plugs from 2 to 3 3. Swapped injectors 4. Compression test 5. Leak down test 6. Changed the spark plug, coil and injectors 7. Looked for vacuum leaks. Mechanic believes that it is related to the cracked cylinder head in the recall, MC-10232269-0001.pdf. My vehicle is just outside of the recall range…
